Connecting a Lowrance Elite 5 DSI to an Icom IC-M422 with the NDC-4 adapter cable. The cable's yellow/transmit to the radio's red/in and cable's orange/receive to radio's white/out appear straightforward.

However the radio has only those two red and white NMEA wires. Is it ok to connect the NDC-4 black wire to the radio's power supply ground wire? Would a fuse in the NDC-4 ground wire provide any protection to the GPS in case of spikes across the vessel ground wiring? Can you offer any suggestions?

The NDC-4 black wire will go to your common ground shared between the power supply, radio and GPS. No need to worry about a fuse for there is no power shared between the connections just data and ground.
